Database Fee - Services Information:
The information provided below describes the fees that will be charged per test for entry into the database. The fee amounts will vary depending on whether your company is using the database administrator (Midwest Toxicology Services, Inc.) as their testing provider or are already set up with a provider for their testing. A portion or each fee described below will go directly to COATS to be used for the advancement of the COATS substance abuse initiative.

Entering "outside test" results into database -$15.95 per test
This fee will be charged to provide for:
  • Entry of test results into COATS database
  • Issuing the COATS card on a negative result
  • Management of SAP providers
    • Set-up of network of local area SAPs (Substance Abuse Professionals) to whom employees who have positive tests will be referred to
    • Verifying credentials of SAPs and place them on an approved list
    • Monitor and periodically evaluate performance of SAPs
    • Verify credentials of any SAP that is used outside of approved list
  • Management of "SAP" referral and return-to-duty process
    • Provide all employees who test positive with referral information to an approved local area SAP
    • Monitor and manage return-to-duty and follow-up testing
      • Verify SAP return-to-duty letter has met COATS program compliance
      • Assist employee with obtaining a qualifying return-to-duty test
      • Notify employee of any follow-up tests that are required and monitor all aspects of completing the test
  • Management of other testing problems or issues
    • Handling diluted test results
    • Handling refusals to test
  • Database development and on-going maintenance
  • Secure website development and on-going maintenance for verifying database compliance and card validity
  • Toll free phone number for verifying database compliance and card validity
  • Administration of audits for owners or companies to verify compliance with COATS program
  • Referrals to other construction related organizations who can provide services to meet COATS program compliance
  • COATS will utilize part of this fee for the advancement of substance abuse prevention and policy as well as for promotion of construction industry safety improvements.
Instructions for entering "outside test" results:
  • Fax a copy of each qualifying test result to 317-262-2222.
  • Each test result must contain
    • Name of the SAMHSA laboratory performing the test
    • Listing of the drug panel with cutoff levels
    • MRO signature verifying review was completed.
  • Each result will be entered into the database if it meets all of COATS testing requirements
  • COATS ID card(s) and invoice will be mailed to the designated contact for the company submitting the test result
Testing services with Midwest Toxicology - $59.95 per test will include the drug test, database entry and COATS card and the following services:
This fee provides for and includes:
  • Drug test to include urine collection, SAMHSA laboratory testing and MRO review of each test
  • Automatic entry of test results into COATS database
  • Automatic issuing of the COATS card on each negative result
  • Management of SAP providers
    • Set-up of network of local area SAPs (Substance Abuse Professionals) to whom employees who have positive tests will be referred to
    • Verifying credentials of SAP and place them on an approved list
    • Monitor and periodically evaluate performance of SAPs
    • Verify credentials of any SAP that is used outside of approved list
  • Management of "SAP" referral and return-to-duty process
    • Provide all employees who test positive with referral information to an approved local area SAP
    • Monitor and manage return-to-duty and follow-up testing

Instructions on how to contact the database to verify employee status in the database or to verify COATS card validity:
  1. Call database toll free at 1-800-358-8450 or 1-317-262-2200
    1. Ask for COATS database
    2. Provide attendant your name and company name
      1. Your company must have previously set you up as a contact eligible to verify card/employee status in the database
    3. Provide attendant with any of the following information to check the status of an employee's testing compliance
      1. Name of employee
      2. Social Security number of employee
      3. COATS card number (this is printed on the card)
    4. The database attendant will provide the following information to you if you are set up to only verify card status
      1. Card is valid
      2. Card in not valid
      3. Card is not on file
    5. The database attendant will provide additional information regarding an employee if your are set up as a DER (Designated Employer Representative) who is allowed to receive confidential information on employee test results and issues related to why a card is not valid.
  2. Fax the database for confirmation of card status - Fax Number: 1-317-262-2222
    1. If you are faxing a list, provide the name, social security number or card ID number of each person you need verification of
      1. Be sure to include the secure fax number you would like the verification faxed back to
  3. Access the database secure web-site to verify employee card status
    1. Each company must set up a user ID number and password to access the database - specific information on the web-site address, etc. will be provided at the time of user set-up
