Submitting Results
For Database Entry

Instructions for submitting "outside test" results:

  • Fax (or provide electronically) a copy of each qualifying test result to 317-262-2222 using the ACI/COATS cover letter (ACI/COATS cover letter submitting «outside test» results pdf format).


  • Each test result must contain:
    • Name of the SAMHSA laboratory performing the test
    • Listing of the drug panel with cutoff levels
    • MRO signature verifying review was completed


  • Each result will be entered into the database if it meets all testing requirements


  • ACI/COATS ID card(s) and invoice will be mailed to the designated contact at the company
