The facilities listed below can be used to locate a counselor that is qualified to perform the SAP evaluation that is required following an ACI/COATS violation (positive test or refusal to test)
Instructions for contacting facility:
  1. Inform the facility that you need to make an appointment to have an evaluation with a counselor who is qualified as a Substance Abuse Professional (SAP) for the COATS program.
  2. Provide information to the facility as to why you need an evaluation (example - you failed a drug test for the COATS program.
