$59.95 per drug test - This fee includes the following services associated with the COATS program:

- Entry of test results into ACI/COATS database
- Issuing a photo ID card on initial negative results
- Employee assistance on policy violations
- Locating & verification of SAP providers
- Management of testing problems and/or issues
- Management of random testing services

- Web access to check employee training
- On-going update & maintenance of database and website

$15.95 per test entry - This fee includes the following services associated with the ACI/COATS program:

| INSTRUCTION FOR SUBMITTING TESTS TO MTS: |
- Fax a copy of each test result to 317-262-2222 with a cover sheet indicating the results are being submitted for the ACI/COATS program.
- The test result must contain the name of the SAMHSA laboratory, drug panel with cutoff levels, and MRO signature verifying review was completed.
- The results will be entered into the database; the ID card(s) and invoice will be mailed to the primary contact listed previously unless other direction is given for mailing cards.
